Privacy Statement and Consent
The Hand Surgery Centre acknowledges and respects the privacy of its clients. The information that you provide is “personal information” as defined by the Privacy Act 1988. This information is being collected for the purpose of processing your patient registration details. The intended recipients of the information are the service providers engaged by the Hand Surgery Centre.
The provision of this information is voluntary but if this information is not being provided, we may be unable to process your client registration. You have the right of access to and alteration of personal information concerning yourself in accordance with the Privacy Act. The information is being collected by the Hand Surgery Centre and will be held by the Hand Surgery Centre.
Privacy Consent
As part of the course of your condition, it is usual to write to your referring Doctor. In the case of compensation matters, it may be necessary to communicate with the Insurers, your Employer and/or Rehabilitation Provider. Your contact and account details may also be provided to a third party for assistance in settling long overdue accounts. Please be advised that any costs incurred in the recovery of outstanding debts will be added to the account. Information will only be released to Solicitors on request, if they can present a separate consent form, signed by you. Clinical photographs may be taken for future treatment or educational purposes.
