• Any relevant test results such as blood tests x-rays, scans, MRI or nerve conduction studies relating to your appointment
• Medicare and Private Health Fund details

• Signed Privacy Consent forms (click here to download)

Workcover

• Doctors referral
• Approval for consultation in writing from your insurance provider
• Employer’s name, phone number, address and contact person
• Date of Injury
• Insurance company name
• Claim number

Third Party

• Insurer Name, phone number, address and contact person
• Date of accident
• Claim number

Non- English speaking patients

• Please bring a family member or friend who can interpret at your consultation.  
• The use of minors as interpreters is not permitted.

If a professional translator or interpreter is needed, please inform our staff when booking your appointment.
